nutritional reboot AGREEMENT
THIS AGREEMENT IS MADE BETWEEN THE Naturopath, BIANCA MARICCHIOLO, ANd YOU THE CLIENT.
Terms
This agreement, will begin upon payment, and will continue for one week after the assessment has been received by the client. The services to be provided to the client is an online nutritional health assessment. The assessment may address specific personal health details, lifestyle, or general conditions in the client's life.
Upon completion of the assessment, the client has unlimited email access to the practitioner for one week. Any further communication can be done in the form of a consultation either via Skype, phone or in person. Your Nutritional Reboot fee is payable upon purchase through the LifeElite website, and a receipt forwarded directly to you, the client.
Confidentiality
The practitioner promises the client that all information provided to the coach will be kept strictly confidential. All matters discussed between us whether in person, via phone, Skype or email, remain fully confidential, records will be kept and disposed of in accordance with appropriate privacy laws.
Nutritional Reboot Assessment Commitment
Once I begin your assessment, I consider myself to be fully committed to you and your health and dietary success. I know you will be committed to you too. This commitment includes you going through the entire assessment from start to finish, to review and seek further clarification from the practitioner is needed.
No part of your assessment is refundable once we commence, however it is transferable to someone else if for any reason you decide not to continue.
Assessment Timings
Your health questionnaire will take you approximately 20-30 minutes to complete. Your Food Diary will take you 1 week to complete. I will start your assessment once both sections have been submitted. If at any time you need to change an answer or recall something, and need to add in , please email me as soon as possible.
The assessment will be emailed back to you within 14 business days. It is very rare for me to run late on this deadline, however if I foresee that this may be a possibility, I will contact you via email as soon as possible.
Contact
I welcome you contacting me via email after you have received your Nutritional Reboot Assessment, and I reply to emails during business hours Monday to Friday. This is a service I provide to all my clients which you are welcome to use and in fact, you will be surprised at how much we can ‘shift, grow, develop and move’ via email alone. Please note that Facebook private messaging is not encouraged as I do not check it often and I prefer to keep all communication on email. Emails will be attended to within a reasonable time-frame during business hours, Monday to Friday.
Health Information and Actions
You acknowledge that the information you submitted is correct and truthful. This includes all answers relating to your medical history, medications, health goals, dietary information and lifestyle. You will not hold me liable for any incorrect information received by the practitioner from you, or loss or cost incurred by you in the event of mental, physical, emotional stress or distress caused directly or indirectly in relation to your assessment. You will not hold me responsible for not achieving, (in whole or part), any of your goals and recognise that outside of the assessment, the dietary recommendations stated can only be carried out by yourself. You will indemnify me in the event of any such claim.
Intellectual Property
You have no right to use or reproduce any of the processes, techniques, presentations, methodologies or materials used by me in your Nutritional Reboot assessment. You must not at any time reproduce materials given to you except for your own personal use.
Declaration
I understand that I am working with Bianca Maricchiolo for The Nutritional Reboot assessment for the specified fee. All information regarding my health and medical history, diet and lifestyle is correct.
